2010 Showcase Registration Instructions and Payment

Thanks for your interest in perfoming in our 2010 Ballroom Showcase Spectacular! 

The deadline for registration and payment is September 6, 2010, but we advise you to register as soon as possible since spots may fill up quickly. 

Following is an explanation of registration and payment options. All participants are required to complete the following steps to register: 

Step 1. Fill out the downloadable Registration Form and return to Alexandra Caluen. The form is editable on your computer, so once you have filled it out you may email it back to Alexandra.  Alternatively, you can print the form and use postal mail or hand deliver it to Alexandra. NOTE: Both Alexandra’s email address and snail mail address are indicated on the Registration Form. A PDF version of the registration form is also available here. 

Step 2: Fill out and sign the downloadable Liability Waiver & Optional Image Release Form. Since this form requires your signature, you will have to either print it and scan it back into your computer to email to Alexandra Caluen, or you can mail it to the address on the Registration form or hand deliver it to her to her. Note: A PDF version of the waiver and release form is also available here. 

Step 3: Pay. Payment can be made online via PayPal using the appropriate button below, or you can mail or hand-deliver payment by check to Alexandra Caluen. NOTE: All registration fees INCLUDE $25 admission to the evening Dance Party and Professional Show. When making your selection, please note there is a separate fee structure for Adult Amatuers and participants under the age of 25.
